TITLE: SCHOOL BASED THERAPIST

REPORTS TO: SCHOOL BASED SERVICES DIRECTOR or SCHOOL BASED SUPERVISOR

STATUS: EXEMPT

MAIN FUNCTION: The School Based Therapist provides mental health and/or substance use services to clients in the school district they are serving, including emergency services. Also, this person provides consultation, education and prevention services to other agencies and individuals.

DUTIES/RESPONSIBILITIES:

Provides individual, family, and group counseling/therapy sessions as assigned.

Maintains up to date client charts to include assessment, social history, psychotherapy notes, treatment plans, screenings, and discharge plans.

Communicates with clients and their support systems to determine the family and community resources available and makes appropriate referrals.

Contacts parent/guardian upon initiation of therapy services and will provide ongoing updates as needed or if there is a safety concern.

Provides crisis intervention and assessment/evaluation, including after hours and on weekends.

Will be part of the on-call weekend rotation schedule to provide 24/7 crisis coverage.

Provides after hours coverage as needed for other programs/therapists within scope of practice.

Provides case consultation as needed.

Provides community education, prevention services, and public relations, as necessary.

Implements and documents new assessments, paperwork and screening tools as directed.

Completes all paperwork in accordance with agency policies and procedures.

Attends supervisory and clinical staff meetings.

Attends school-based meetings including, but not limited to, 504s, IEPs, safety planning/re-entry meetings, and behavior meetings.

Attends required professional development training within the school.

Assists in quality assurance.

Meets expectations for monthly billable baseline.

Participates in a planning committee as assigned.

Participates in community events as assigned.

Provides services at other HCS locations as assigned, which may include travel to provide services.

Performs other related duties as assigned.

Participates in collection of mental health and substance use information and student outcome data.
