Concern Housing is seeking a Medication Manager to be responsible for providing oversight and monitoring for the supportive tenants’ medications; education regarding medications; linkage with appropriate resources and supportive case management services.

Days/Hours: Monday - Friday; 9am - 5pm

Duties and Responsibilities:

Provide medication monitoring at the residences of identified clients who are in need of this service, up to daily, as needed.

Assess client needs to determine the frequency of medication monitoring.

Provide education and resource information regarding prescribed medication regimens to increase medication management skillsets.

Ensure an adequate and appropriate supply of mediations are maintained at the residence.

Ensure adequate and appropriate supplies are on hand at client residences, i.e., personal refrigerator with thermometer, sharps container, lock box, as needed.

Provide oversight and support for clients who pack medication according to their personalized medication hold schedules.

Assess and help prepare clients to manage their medications independently, providing: counseling on monitoring the medication supply, education on how to order refills, and linkages to resources to support in this area.

Responsible for all documentation regarding support services, including: drafting Support Plan Goals, Objectives and Staff Interventions; providing a medication management (MMT) restorative service once monthly and documenting in Precision Care.

Ensure current providers/prescribers are accurately listed on client Face Sheets; ensure appropriate consents are on file.

Conduct and document collateral contacts with providers regarding any medication refusals, misses, script renewals, etc.

Ensure client medication lists are accurately reflected in Precision Care and review/update at least quarterly.

Review hospital discharge paperwork to monitor for any potential medication changes; follow-up with providers/prescribers as needed.

Attend weekly supervision and scheduled staff meetings.

Remain current with all agency required compliance documents and trainings.

Requirements: Bachelor’s Degree in Psychology, Rehabilitation, Social Work or other related mental health discipline, plus 2 years experience working with a psychiatric population. A working knowledge of psychotropic medications is also required. This individual must have good verbal and written communication skills as well as the ability to make sound judgments regarding client care.
