Salary: $22-$26

Position Title: ConnectedCare Case Manager

Department:Homeless Guest Services

Reports To:Director of Homeless Guest Services

Status:Full-time, Non-Exempt, Hourly

General Function:The Connected Care Case Managerwill develop andfacilitateindividualized case management services to reduce shelter stays and connect guests with other supportive services.

Principal Activities:

Provide individualized case management toConnected Care clients.

Work with community agencies, SBRM Mens Program, and HGS to coordinate intakes & exits of Connected Care Beds

Develop a clearly writtenindividualized treatment plan for assigned caseload

Maintainaccuratedata in computer databases and logs

Build trust and rapport with guests

Provideappropriate referralsincluding(but not limited)to substance abuse treatment

Assist ConnectedCare clientswith transportation needs

Trackclientprogress and provide updates and report to HGS team and other SBRM staff

Utilize a client-centered approach that includes motivational interviewing, harm reduction, and trauma informed care

Provide crisis intervention services when needed

Maintain a current, thorough knowledge of community resources andutilizethem to provide comprehensive, wrap-around services to clients

Participate in evaluation and trainings to better respond to client/community needs

Support Shelter Case Managers as needed

Perform case management activities including (but not limited to):

Intake assessments and evaluations

Establishing program goals and priorities.

Conducting scheduled case management meetings with guests.

Following up on treatment referralsfor clients and supporting clients transitionfrom ConnectedCareBed to treatment

Be a contributing member of the Homeless Guest Services team.

Meet with Homeless Guest Services team to discuss operations and guests

Keep team apprised ofclientissues and concerns

Maintain good rapport with Mensstaff, HGS staff, andsecurity personnel with a willingness to work together to solve difficult problems

Act as a responsible party, alongside SBRM Mens Staff, HGS Staff,andsecurity personnel, for any problems, emergencies, or disputes that can arise while guests are on SBRM property

Support the Emergency Shelter Team as needed

Assistwith piloting new Homeless Guest Services initiatives and programs

Compensation and Benefits:

$22.00-$26.00 DOE
