About the Role:

The Case Manager directly partners with clients within various Center for Hope programs, providing support and guidance in reaching client goals. The Case Manager receives referrals from Center for Hope partners, including government and community agencies. This position is responsible for conducting client and family needs assessments, linking to indicated services or programs and providing a warm hand-off, helping clients to complete service and program applications, conducting home and community visits, conducting workshops and support groups, liaising between client and community resources, accompanying clients to court dates and other hearings, and conducting various forms of community outreach and education. The Case Manager is also responsible for other pertinent duties as assigned by the Program Manager.

KEY RESPONSIBILITIES:

The Case Manager will provide case management services to clients and families participating in the Center for Hope program.

The Case Manager will be working in the community, doing home visits, visiting schools, participating in community events and focusing on victims impacted by community violence. Hours can vary due to various events.

Receive client and family referrals and conduct initial needs assessments.

Partners with clients in case planning and goal setting, continuously evaluating and recording progress made towards goals.

Provide linkages to appropriate services and programs with a warm handoff.

Assist in vetting various community programs and agencies to ensure a trauma-informed and culturally appropriate environment and service delivery.

The Case Manager will conduct support groups and seminars on literacy, job readiness, anger management, resilience, coping mechanisms, conflict resolution, family engagement, parent-child bonding, and other pertinent topics.

Conduct presentations and trainings to community partners – agencies and community members – on interpersonal dynamics, trauma, program subject matter expertise, trauma-informed practices, culturally appropriate responses.

Partner with Center for Hope programs to promote programming at community events and other gatherings.

Attend various community events on behalf of Center for Hope and build relationships with community members, agencies, and other stakeholders.

Conduct presentations and trainings at various community organizations and for community groups on interpersonal dynamics, trauma, program subject matter expertise, trauma-informed practices, culturally appropriate responses, and Center for Hope Programming.

REQUIREMENTS:

Education: Bachelor's degree required in Social Work, Human Services, Family Science, Psychology or other similar fields of study

Experience: 1-3 years of experience. Travel is required to homes in the community, schools, community events etc. Mileage will be reimbursed. Hours can vary due to various events.

Grant supports up to $24/hour
