Job Summary

We are seeking a full-time LVN Case manager to work in our office in Arcadia, CA as part of an integral hospice team. The ideal candidate for this position encompasses an overall understanding of hospice and its day to day operations. Case manager duties include assessing, intake, planning, implementing, monitoring and evaluating actions required to meet the patient's health and human services needed. The LVN's main role is to expedite and enhance the quality of patient management, maximize satisfaction for our patients and families and promote quality assurance.

Responsibilities

Coordinate with colleagues within the office and field to assess, plan, implement, or evaluate patient care plans.

Effectively coordinate the intake process of newly admitted patients.

Ensure all orders are received and processed timely for patient delivery.

Communicate effectively with hospice team regarding patient needs and facilitate multiple care aspects.

Participate in hospice IDT meetings as part of an interdisciplinary team of Nurses, Physicians, Social Workers and Chaplain.

Record information with accuracy and produce reports as needed.

Provide compassionate and caring interaction with patients/families to ensure satisfaction.

Promote high quality and cost-effective interventions and outcomes.

Maintain accurate records of patient care and discuss observations with the supervisor.

Adhere to professional standards as outlined by protocols, rules and regulations.

Qualifications

Currently licensed as a Vocational Nurse in state of California

Previous experience as an LVN case manager is preferred

Technical skills needed for Microsoft, Excel, Google Docs and EMR system

Excellent interpersonal and problem solving skills

Strong organizational and time management skills

Ability to multi-task and work as part of an interdisciplinary team

Compassionate and able to discuss difficult topics related to end-of-life

Determined and interested to learn and grow within hospice
