JOB TITLE - LICENSED SOCIAL WORKER
GENERAL DESCRIPTION - Provide social services to residents, their families, and significant others enabling them to deal with the impact of illness and extended care placement while achieving maximum benefit from health care services.
TYPICAL DUTIES/RESPONSIBILITIES -
1. Assist in meeting the psychosocial needs of each resident, their families, and significant others as appropriate or necessary in coping with the effects of illness, disability, treatment, or extended care. The specific services include, but are not limited to, the following:
A. Provide individual, family, and group services designed to improve social functioning and reduce the psychosocial problems of residents.
B. Contribute to an environment designed to enhance positive self-images of residents and preserve their human dignity.
C. Advocate in cooperation with other facility personnel the rights of each resident.
D. Assist residents, families, and others as necessary and appropriate in finding and utilizing financial, legal, mental health, and other community services and resources.
E. Assist in obtaining guardianship and conservatorship services for residents needing these services.
2. Review social service policies and procedures at least annually.
3. Provide group services on a regular basis including Resident Council meetings once a month.
4. Identify the medically related social and emotional needs of all residents.
5. Coordinate residents, family, staff, and community resources to meet each resident's special needs while promoting individual well-being.
6. Preside over pre-admission and admission meetings with families and prospective residents.
7. Maintain and utilize a comprehensive community resource file for finding and inviting prospective residents.
8. Provide crisis intervention for residents and families.
9. Document interventions and assessments in resident charts.
10. Provide an avenue for family and resident complaints and procedures for resolutions.
11. Participate in resident care management:
A. Participate in regularly scheduled resident care conferences.
B. Assist in development of multi-disciplinary care plans.
C. Review and revise the plans for social service intervention as necessary.
D. Make appropriate referrals for services such as dental, eye, and ear tests and care.
E. Obtain from other agencies including physicians, information pertinent to resident evaluation and development of an appropriate social service care plan.
12. Assure that each resident and/or responsible party is informed of his/her rights upon admission, at the time of any changes, and at least annually thereafter.
13. Be responsible for performing and coordinating with staff all admissions, discharges, and room changes.
14. Conduct an annual all-staff in-service on resident rights, open admission, advanced directives, and confidentiality, and update staff on any changes in these areas.
15. Be responsible for identifying inappropriate, abusive behavior to residents such as derogatory language, rough handling, etc. and referring the situation to a suitable staff as deemed appropriate for the incident.
16. Complete other duties as assigned by the Administrator.
JOB RELATIONSHIP - accountable to the administrator; must cooperate with other staff to make the facility a friendly home for the residents and their families.
QUALIFICATIONS -
1. Graduate of an accredited social work curriculum leading to a Bachelor of Social Work.
2. Must be a Licensed Social Worker.
JOB KNOWLEDGE/SKILLS REQUIRED -
1. Working knowledge of resident assessments, care plans, RAPS, discharge summaries.
2. Skills in working with people to be able to implement as smooth transitions as possible for the residents and families with problems concerning admission and living in a skilled care facility.
3. Knowledge of laws and regulations concerning resident rights and all aspects of living in a nursing facility.
4. Knowledge of and ability to abide by personnel issues as outlined in the Wentz Living Center Employee Handbook.
5. Ability to document incidents and prepare social histories in the charts for each resident.
6. Ability to realize what has to be done and work to accomplish these goals without supervision.
7. Ability to prioritize tasks to accomplish most important goals first.
8. Ability to keep in confidence staff and resident information.
PHYSICAL REQUIREMENTS -
1. Ability to sit, stand, and walk intermittently during a shift.
2. Ability to type or enter data on a computer.
3. Ability to bend/stoop, squat, kneel, reach above shoulders, and lift and carry up to 10 pounds occasionally.
