Licensed Practical Nurse (LPN) - Long-Term Care Team All Shifts

Countryside Care Center Rehab & Nursing Facility is seeking a Licensed Practical Nurse (LPN) to join our long-term care team in a full-time, part-time, or PRN capacity.

This role focuses on delivering practical nursing care that supports resident comfort, clinical stability, and overall quality of life. You'll work closely with nursing leadership and interdisciplinary staff to ensure consistent, high-quality outcomes across the unit.Key Responsibilities:

Provide direct nursing care aligned with individualized care plans

Administer medications and treatments per physician orders

Observe and report changes in resident condition

Collaborate with RNs, physicians, and care team members

Maintain timely and accurate clinical documentationCompensation:

$32.00 - $39.42 per hour

Total earning potential based on experience and benefit selections

Base pay adjusted according to qualifications

Employees who decline certain benefits may qualify for higher hourly ratesQualifications:

Active LPN license in good standing

At least 1 year of nursing experience preferred

Strong organizational and communication skills

Ability to work effectively in a team-based care setting
