Overview

Licensed Practical Nurse (LPN) – Long-Term Care in White River Junction, Vermont. Travel temporary assignment beginning 05/04/2026 for a duration measured in weeks, with 40 guaranteed hours per week. Evening and night shifts available in a LTC/assisted living setting. This travel opportunity is being staffed through Healthforce Healthcare LLC.

Responsibilities

- Administer medications and treatments as prescribed; monitor and respond to patient needs.

- Provide direct patient care, including assistance with activities of daily living, mobility, hygiene, and nutrition support.

- Monitor vital signs, assess for changes in condition, and escalate concerns to the supervising RN promptly.

- Assist with care planning and implementation in collaboration with the interdisciplinary team.

- Maintain accurate documentation in the electronic health record and ensure compliance with privacy and safety standards.

- Uphold infection control practices and maintain a safe, comfortable environment for residents.

Requirements

- Active LPN license in Vermont (in good standing).

- Ability to work evening and night shifts; flexible to meet assignment needs.

- Experience in long-term care or assisted living settings preferred.

- Strong communication and teamwork skills; proficient in documentation.

- Ability to pass background checks, health screening, and meet onboarding requirements for travel assignments.

- Capable of adapting to a fast-paced, variable schedule in a temporary assignment.
