Responsibilities

- Provide direct nursing care to residents per individualized care plans and facility policies

- Administer prescribed medications and document administration accurately

- Monitor residents’ condition, vital signs, and respond to changes in status

- Assist residents with activities of daily living and ensure comfort and safety

- Collaborate with the interdisciplinary team to coordinate care and communicate changes

- Maintain accurate documentation in the medical record and respond to routine and emergency calls

- Promote a safe environment, adherence to infection control practices, and resident rights

Requirements

- Active Licensed Practical Nurse license in Vermont

- Education: graduate of an approved LPN program

- Experience: 1+ year of nursing experience in long-term care or related setting preferred

- Ability to work day and evening shifts in a 40-hour weekly schedule

- Comfortable working in a long-term care / assisted living setting and following care plans
