What Youll Do

Provide hands-on nursing care focused on comfort and quality of life

Administer medications and monitor patient responses

Observe, document, and report changes in condition

Collaborate with RN Case Managers and interdisciplinary team

Support patients and families with compassionate communication

Follow individualized care plans and ensure HIPAA compliance

What Were Looking For

Active Ohio LPN license (required)

1+ year of nursing experience (hospice or home health preferred)

Strong clinical and documentation skills

Reliable, compassionate, and patient-focused

Ability to work independently and as part of a team
