Overview

Licensed Practical Nurse (LPN) – Long-Term Care

Claremont, New Hampshire, United States

Travel - Temporary Assignment Start: 04/21/2026 Duration: 13 weeks

Weekly pay: $1,565-$1,633 per week Shift: Evenings (5x8) Facility type: LTC - Nursing Home

This travel assignment requires the candidate to have their own transportation due to a rural setting with limited housing options and public transit.

- Travel assignment in a rural LTC setting

Responsibilities

- Observe residents continuously, note changes in mental and physical status, inform the Supervisor, and provide detailed status reports at shift change

- Receive, transcribe, and implement physician orders; administer medications and treatments; document MARs/TARs and electronic records; manage unit pharmaceutical supply

- Supervise Licensed Nursing Assistants in accordance with NH Board of Nursing rules; assign care tasks and assist staff as time allows

- Accompany physicians on rounds on assigned units; facilitate communication during 60-day resident reviews

- Administer oxygen per facility guidelines; perform gastrostomy/jejunostomy feedings per policy; provide wound care including wound vacs

- Perform routine unit duties: review appointments, therapy schedules, activity board, ambulation list, vitals, weights, and nutritional documentation; ensure infection control practices and unit safety

- Manage admissions, discharges, and transfers; complete 24-hour unit reports; review new orders and coordinate with Nurse Manager/Nursing Supervisor

- Communicate with residents’ families regarding orders and needs; receive calls and determine appropriate responses

- Maintain accountability for the unit during shift leadership; follow regulations for storage and documentation of controlled medications

- Support orientation and education of new nursing staff; participate in mandatory staff meetings and contribute to ongoing staff development

Requirements

- Current, active Licensed Practical Nurse license in New Hampshire

- 3+ years of current nursing experience within the last 6 months (required)

- IV therapy certification (must be willing to obtain during onboarding if not certified)

- Willingness to float to units where experienced

- Covid-19 vaccination required (exemptions not accepted)

- Own transportation required; rural location with limited housing/public transit

- Local candidates within 50 miles of the facility will not be accepted

- BLS (AHA) certification required

- Prior travel nursing experience strongly preferred

- Rural/small facility experience strongly preferred

- Education: graduate of an approved practical nursing program; able to perform duties per state scope of practice
