This travel assignment is a licensed practical nursing role in a long-term care environment. Responsibilities

- Administer prescribed medications and treatments according to physician orders and facility policies.

- Monitor residents for changes in condition, perform assessments, and document findings for the care team.

- Assist with activities of daily living (ADLs) and provide compassionate resident care.

- Perform routine nursing procedures within scope of practice and maintain accurate and timely charting.

- Collaborate with RNs, physicians, and other disciplines to develop and update individualized care plans.

- Uphold safety, infection control, and resident privacy standards; report concerns promptly to the supervising clinician.

Requirements

- Active Licensed Practical Nurse license in Vermont (in good standing).

- Graduate of an accredited practical nursing program.

- Current CPR/BLS certification.

- Prior experience in long-term care or skilled nursing settings preferred.

- Ability to work evenings and nights and meet the demands of a 40-hour workweek.

- Strong communication and documentation skills; able to work effectively within a multidisciplinary team.
