Weekly Hours: 48 hours

Schedule: Flexible; must be available for variable shifts

Position Overview

"Navitas Healthcare, LLC" is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide high-quality care in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting. The LPN will work under the direction of the RN or Nursing Supervisor to ensure resident safety, comfort, and adherence to individualized care plans.

Responsibilities

Administer medications and treatments as prescribed.

Monitor, record, and report changes in residents' conditions.

Assist with daily living activities, including hygiene, mobility, and nutrition.

Collaborate with interdisciplinary teams to develop and implement care plans.

Maintain accurate and timely documentation in resident medical records.

Respond to emergencies and provide first-line interventions as needed.

Educate residents and families on care routines and health management.Required Qualifications

Current LPN license in the state of practice.

Basic Life Support (BLS) certification.

Valid driver's license.

Covid-19 vaccination card.

Updated professional resume.Education

Completion of an accredited Licensed Practical Nurse program.Licensure / Certification

Minimum 1 year of experience in a SNF, LTC, or similar clinical setting preferred.

Familiarity with electronic health record (EHR) systems.

Strong understanding of nursing procedures and safety protocols.
