Physical Therapist (PT) – Home Health

Location: Salem, OR,

Opportunities in Eugene, McMinnville & Corvallis are also available.

Work Type: Full-Time (Part-Time & Per Diem Options Available)

Compensation: $62- $70/hour depending on experience, milage reimbursement

About the Opportunity

A respected, mission-driven home health organization is seeking an experienced and compassionate Physical Therapist to join its expanding care team in the Salem, OR region. This role offers the unique opportunity to provide one-on-one, patient-centered care in the comfort of patients' homes while enjoying schedule flexibility and the autonomy to manage your own caseload.

As a Physical Therapist, you will assess patient needs, develop personalized treatment plans, and support individuals as they work toward improved mobility, strength, and independence. You will collaborate with a supportive interdisciplinary team and contribute to high-quality, outcome-driven home health care.

Key Responsibilities

Evaluate patients' physical condition, functional abilities, and mobility needs

Develop and implement individualized therapy treatment plans

Educate patients and caregivers on therapeutic exercises, safety practices, and care strategies

Document care accurately and in a timely manner in the EMR system

Coordinate care with nursing, occupational therapy, and other clinical partners

Travel within the assigned territory to provide in-home visits

Required Skills

Active Physical Therapist license in Oregon

Doctorate or Master's degree in Physical Therapy

Minimum 1 year of experience as a Physical Therapist

Home health experience strongly preferred

Current BLS / hands-on CPR certification

Reliable transportation with valid driver's license and auto insurance

Additional Skills to Consider

Experience with EMR systems

Familiarity with Medicare home health regulations

Ability to manage a home-based caseload efficiently

Skill in developing functional, goal-oriented treatment plans
