Responsibilities

- Provide skilled nursing care to residents in a long-term care setting under supervision of an RN or physician.

- Administer prescribed medications and treatments as ordered; document administration accurately.

- Monitor resident health, vital signs, and changes; respond to alarms and report significant findings to the supervising clinician.

- Perform routine assessments, assist with care planning, and support wound care and skin integrity as delegated.

- Assist residents with activities of daily living, nourishments, hydration, mobility, and fall-prevention activities.

- Maintain accurate documentation in the electronic health record; adhere to infection control and safety standards.

Requirements

- Active Licensed Practical Nurse license or eligibility for licensure in Vermont.

- Graduation from an accredited practical nursing program.

- Basic to intermediate computer/charting skills; ability to document in the electronic health record.

- Ability to work day or evening shifts and meet 40 hours per week; eligible for a travel assignment.

- Experience in a long-term care or geriatric nursing setting preferred.

- Strong communication and teamwork skills; commitment to resident-centered care.
