Trinity Rehab Services is hiring a Physical Therapist Assistant to work across Skilled Nursing and Outpatient services. You’ll join a collaborative, person-centered team with strong onsite support and realistic expectations.

What You’ll Do:

Provide PTA services under established plans of care

Treat patients in SNF and/or Outpatient settings as appropriate

Collaborate closely with an interdisciplinary team

Deliver individualized, meaningful patient care

Complete documentation efficiently and accurately

What We’re Looking For:

IL Licensed Physical Therapist Assistant (PTA) or Pending licensure

New graduates encouraged to apply

Strong communication and teamwork skills
