Job Title: Licensed Practical Nurse (LPN) – Long-Term Care
Location: Falmouth, MA 02540
Pay Rate: $50.00 – $60.00 per hour
Schedule: Full-Time 40 Hours per Week
Shift Details:
Day Shift: 7:00 AM – 3:30 PM
Evening Shift: 3:00 PM – 11:30 PM
Rotational weekends and holidays requiredPosition Overview
"Navitas Healthcare, LLC" is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join a Long-Term Care (LTC) facility. In this role, you will provide direct patient care, support resident well-being, and supervise nursing assistants while ensuring compliance with healthcare regulations. This position requires strong clinical skills, attention to detail, and the ability to work collaboratively within a healthcare team.Key Responsibilities
Deliver direct nursing care in accordance with care plans and physician orders
Supervise and guide nursing assistants in daily patient care activities
Administer medications and treatments safely and accurately
Monitor, assess, and document resident conditions and responses to treatment
Maintain accurate and timely medical records, including admissions, transfers, and discharges
Communicate effectively with physicians and interdisciplinary teams
Participate in care planning and physician rounds
Perform clinical procedures such as wound care, catheterization, tube feeding, and specimen collection
Record vital signs and report changes in patient condition
Ensure adequate medical supplies and equipment availability
Educate residents and families on health-related matters
Support compliance with all regulatory standards and facility policies
Assist with new resident orientation and provide exceptional patient-centered careRequired Qualifications
Education:
Completion of an accredited LPN program or Nursing Degree
Licensure/Certifications:
Active, unencumbered LPN license (Massachusetts)
BLS / CPR / AED certification
Experience & Skills:
Knowledge of long-term care practices and healthcare regulations
Strong leadership and supervisory skills
Excellent communication and interpersonal abilities
Ability to make sound clinical decisions independently
Proficient in documentation and patient record management
Team-oriented with a compassionate, patient-first approach
