Primary Care - Nurse RN - Hybrid-Mercy Family Medicine Strafford

Under the direction of the primary care operations team, the Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel. Duties and responsibilities will be performed in a manner consistent with our mission, values, and Mercy Service Standards.

Position Details:

This is a full time hybrid position

Schedule: 8a-5p; Monday through Friday

90 days of onsite training then 1-2 days onsite

LPNs are welcome to apply!

Minimum Requirements:

Education: Diploma in Associate of Applied Science in Registered Nursing

Licensure: Active RN licensure

Preferred Requirements:

Education: Bachelor of Science in Nursing
