Weekend Premium Shift Differentials

• Evenings and Nights (Fri–Sun): Base Rate ($30/hr) + $9.50 per hour, up to $39.50

• Days (Sat–Sun): Base Rate ($30/hr) + $7.00 per hour, up to $37.00

Licensed Practical Nurse (LPN) Responsibilities

• Administer medications and treatments as prescribed

• Monitor residents and report changes in condition

• Assist with admissions, transfers, and discharges

• Maintain accurate documentation in accordance with regulations

• Follow infection control, safety, and facility policies

• Collaborate with the interdisciplinary team to ensure quality outcomes

• Provide compassionate care to residents requiring long-term or rehabilitative services

Licensed Practical Nurse (LPN) Qualifications

• Active LPN license in New York State

• BLS certification required

• Skilled nursing or long-term care experience preferred

• Strong communication and teamwork skills

• Ability to work in compliance with NYS Nurse Practice Act and facility policies
