Multi-Facility MDS Nurse (RN) - Hybrid

Location: Eastern Massachusetts

Schedule: Full-Time Hybrid (Remote + On-Site Support)

Salary: $115,000 + Full Benefits

We are seeking a skilled MDS Nurse (RN) to support clinical assessment and reimbursement processes across several skilled nursing facilities in Eastern Massachusetts. This role is ideal for an experienced MDS professional who enjoys variety, autonomy, and collaborating with different care teams.

This position combines remote work with routine on-site presence (approximately 1-2 days per week).Core Functions

Manage the completion of MDS assessments with accuracy and timeliness

Perform resident interviews and gather clinical data to support assessments

Partner with interdisciplinary teams to build and maintain individualized care plans

Audit assessments for regulatory compliance and documentation standards

Contribute to reimbursement integrity through PDPM and case mix (CMI) awareness

Support Quality Measures (QM) and Quality Reporting Program (QRP) performance

Step into facilities as needed to maintain continuity of assessment processesWhat You Bring

Active RN license in Massachusetts (or ability to obtain)

2+ years of MDS experience in a skilled nursing environment

Solid understanding of PDPM, and related reimbursement practices

Ability to adapt quickly in different facility settings

Strong attention to detail and time management skills

Clear and professional communication style
