Job Title: Case Manager / Coach (Social Services)

Location: Columbus, OH

Pay: $30–$35/hour

Job Type: Full-Time or Part-Time

About the Role

We are seeking a compassionate and motivated Case Manager / Coach to join our Programs & Client Services team in Columbus, OH. In this role, you will provide individualized support, coaching, and service coordination to participants enrolled in impactful community-based programs. Key Responsibilities

Participant Support & Coaching

Build strong, goal-oriented relationships with participants

Conduct assessments to identify needs, strengths, and barriers

Develop and support individualized service/action plans

Provide ongoing coaching, motivation, and accountability

Case Management & Coordination

Manage an active caseload and track participant progress

Coordinate referrals to community resources and internal programs

Advocate for participants in employment, education, behavioral health, and reentry services

Monitor follow-ups and service utilization

Documentation & Compliance

Maintain accurate and timely case notes, service plans, and reports

Ensure documentation meets program and funding requirements

Support data tracking and reporting efforts

Collaboration & Teamwork

Work closely with Program Coordinators, Clinical Supervisors, and team members

Participate in team meetings, case reviews, and supervision

Communicate participant progress and challenges effectively

Qualifications

High School Diploma or GED required; Associate’s or Bachelor’s degree preferred in Social Work, Human Services, or related field

Experience in case management, coaching, reentry, youth development, or social services preferred

Strong interpersonal, communication, and organizational skills

Ability to manage multiple cases and maintain detailed documentation

Knowledge of community resources and referral systems is a plus
