Job Description

Thorek Memorial Hospital is a stable community hospital with its roots in Chicago for over 100 years. We are committed to excellence in patient care, serving our community, and committed to the continued learning of our employees. We are currently seeking a Casual Nurse Practitioner.

Responsibilities:

Provide direct primary health care to Nursing Home patients, in collaboration with consulting, to include identification, management, and/or referral of health problems. Conduct comprehensive or episodic health assessments and develop patient care plans within clinical nursing practice and medical care protocols. Take patient histories, conduct physical examinations, and record the date in the patient's record; screen patients to determine the need for hospital admission, making referrals to other disciplines as indicated, write progress notes; prepare patient workups and summaries noting pertinent positive and negative findings; obtain psychosocial history relevant to patient care, and assume responsibility for developing therapeutic relationships with patients.

Job Requirements:

Nurse Practitioner Program and certified by the American Nurses Credentialing Center or the American Academy of Nurse Practitioners

The employee shall meet the requirements of the State of Illinois Board of Medical Examiners; and be licensed to practice medicine without restrictions within the State of Illinois

Current licensure as an NP to practice in the State of Illinois

Current DEA certification

Current CPR certification required

Current ACLS certification

Must be able to meet and maintain current Methodist Hospital of Chicago and Thorek Memorial Hospital credentialing and privileging requirements.

Must exemplify interpersonal and customer service skills with the ability to deal with people professionally in a variety of situations

Must be willing to work flexible scheduling needs and be willing to accommodate physicians schedule

Must be willing to travel between both locations

Must be willing to work evening hours and weekends if necessary
