Bachelor-level starting pay $22.34 an hour with $1000 sign-on bonus
Associate-level starting pay $18.87 per hour with $1000 sign-on bonus

Regular supervision What You'll Do
Case Managers help youth, adults, and families in our community live healthier and more fulfilling lives through identifying and supporting their needs and goals.
Case Managers specialize in:
Managing the care provided to youth, adults, and families
Helping youth, adults, and families gain access to the services they need to meet their goals
Further specialized case managers that work with DCS and probation or provide Wraparound Primary duties include:
Evaluating the consumer's need for services
Developing and monitoring a treatment plan that supports the consumer's goals
Helping the consumer obtain services needed to support their goals, such as food or housing assistance, medical care, Social Security benefits, transportation, skills training, etc.
Following up to make sure the consumer is receiving services and those services are helping
Advocating for the family and serving as a link between DCS, court, schools, healthcare providers, employers, community agencies and other Oaklawn services
Documenting all consumer interaction electronically
Interacting directly with consumers (face to face - frequency varies with need, availability of the consumer and the level of intensity)
Interacting frequently with outpatient therapists, physicians, other employees involved in the case, community agencies, referrals sources and other services
Caseload expectations will take into account training, case complexity, level of frequency and/or other assigned duties
Hours will include availability in the evenings and/or weekends Specialized positions do all of the above as well as:
Work directly with probation and Department of Child Services
Parent education and supervised visitation
Provide high fidelity Wraparound Minimum requirements:
BA/BS in social service or a human services-related field and two years' experience in a social service setting. In lieu of bachelor degree, an associate degree and two years' experience providing direct casework to children and families
Higher starting pay based on qualifications
