Bachelor's degree in human services, Psychology, Social Work, Sociology or related field
Computer proficient
Experience with Electronic Health Records and ability to do concurrent and timely documentation preferred.
Ability to communicate effectively both in person and on the phone.
Strong attention to detail
Valid driver's license with ability to travel throughout Bucks County.

Work within a dynamic team of professionals dedicated to compassionate care!
Provide case management for adult individuals diagnosed with severe and persistent mental health issues, including coordinating, and monitoring the activities of the individual's treatment plan; provide individual/group support and symptom management; and advocate for individuals rights and preferences using the recovery model in the ACT program.
Provide on-call crisis intervention services covering evening, nighttime, weekend, and holiday hours and serve as backup to primary, on-call staff.
Consult with community agencies and families to maintain coordination in the treatment process.
Carry out case management, treatment rehabilitation, and support services.
