As a Home Health LPN, a vital part of the patient's care team is provided by the employee, offering skilled nursing care and support. This includes tasks such as:

Monitoring vital signs

Assisting with wound care and personal care

Educating patients and families

Maintaining medical records

Collaborating with other healthcare professionals. Also, provide emotional support and companionship, building genuine connections with patients. Qualifications for the Licensed Practical Nurse LPN position:

Graduate of state-approved school with course in pharmacology

Current license in the State of Pennsylvania

Reliable, independent means of transportation to make home visits. Drivers must have a valid driver's license and drive a vehicle with insurance limits as established by the state.
