Nurse – Residential Services Program

Position Summary

The Nurse for the Residential Services Program is responsible for ensuring that all healthcare-related policies, procedures, and practices within the group home align with best practices, regulatory standards, and licensing requirements. This role provides direct and consultative nursing services, supports staff through training and guidance, and advocates for the physical and medical well-being of residents.

Minimum Qualifications

Current LPN or RN licensure in the State of Ohio

Minimum of one (1) year of nursing experience

Training and/or experience working with traumatized adolescents preferred; willingness to complete trauma-informed care training required

Strong verbal and written communication skills

Experience with adolescent populations and psychiatric medications preferred

Flexibility and adaptability in a residential treatment setting

Valid Ohio Driver's License with insurable driving record

Agreement with Central Ohio Youth for Christ (COYFC) and Gracehaven Standards of Conduct and Statement of Faith

Key Objectives

Foster a safe and supportive environment for staff and residents

Assess, document, communicate, and monitor implementation of each resident's healthcare goals within the Individualized Service Plan (ISP)

Evaluate medical-related policies and practices for compliance

Identify and address healthcare training needs for staff

Maintain compliance with nursing licensure and regulatory requirements

Participate in Gracehaven events and supervisory/team meetings

Essential Duties and Responsibilities

Ensure a Safe Environment

Promote personal safety and a safe environment for residents and staff

Perform duties in accordance with agency policies, procedures, professional, and community standards

Maintain healthcare-related supplies and inventory as needed

Assess, Document, and Implement Resident Healthcare Plans

Ensure systems are in place to meet residents' physical healthcare needs, including triage, direct services, care coordination, and communication

Conduct healthcare assessments at admission and ongoing; contribute to and monitor the medical components of each resident's ISP

Evaluate progress and recommend adjustments, communicating changes to the treatment team and leadership

Review ISPs and Behavior Intervention Plans (BIPs) for medical contraindications

Maintain working knowledge of resident health needs, including nutrition, weight, allergies, medication education, side effects, risks, and benefits

Manage medication acquisition, storage, administration, and documentation in accordance with physician orders

Conduct quality assurance reviews of prescriptions, discharge summaries, and medication administration records

Train staff on resident-specific medical needs, medication handling, and monitoring of self-administration

Assist with medical exams, procedures, intake assessments, and post–unauthorized leave evaluations

Provide wound care, emergency care, or referrals to emergency medical services as needed

Assist in developing safety plans for residents with medical or behavioral risks

Ensure safe storage and disposal of unused or expired medications

Maintain accurate, timely, and compliant medical records within the EMR and resident binders

Advocacy and Collaboration

Collaborate with the Director of Residential Services, physicians, psychiatrists, pharmacists, clinicians, custodial agencies, and case managers

Coordinate healthcare services with external providers and family members as appropriate

Provide health and wellness education to residents and families

Advocate for residents' medical needs within interdisciplinary teams

Participate in monthly staffing and quarterly ISP reviews, providing professional healthcare input

On-Call Responsibilities

Participate in 24/7 on-call rotation for medical escalations

Respond via phone, telehealth, or in person as required

Coordinate with nursing and medical services to ensure continuous coverage

Compliance and Professional Standards

Maintain active nursing licensure and required certifications, including QBHS/QMHS

Complete ongoing training to meet licensure and agency requirements

Ensure compliance with HIPAA privacy and security regulations

Organizational Participation

Attend Residential Services Team meetings and all-staff meetings

Participate in bi-weekly supervision

Assist with Gracehaven fundraising and organizational events
