Position Overview
We are seeking a dedicated and patient-focused Care Manager RN – Emergency Department to provide comprehensive care management services for at-risk patients in an acute care environment. This role plays a critical part in assessment, care coordination, resource utilization, and discharge planning to ensure safe, efficient, and high-quality patient outcomes.
The Care Manager RN collaborates closely with physicians, nursing teams, care management assistants, and leadership to support continuity of care while advocating for patients and families in a fast-paced emergency and inpatient setting.
Key Responsibilities
Assess, plan, and manage care for at-risk patients in acute care settings
Coordinate multidisciplinary care and optimize resource utilization
Develop and facilitate discharge planning and follow-up to support continuity of care
Accurately document patient interventions and maintain regulatory compliance
Serve as a patient advocate and provide education to patients and families
Collaborate with physicians, nursing staff, and interdisciplinary healthcare teams
Promote patient-centered, whole-person care
Participate in professional development and care management programs (e.g., TIPS or Case Management Orientation)
Required Qualifications
Associate’s Degree in Nursing (ADN) or Nursing Diploma upon hire
Active Washington State Registered Nurse (RN) License
Minimum of 2 years of acute care experience in a Medical-Surgical or Inpatient setting
Preferred Qualifications
Bachelor’s Degree in Nursing (BSN) or higher (to be obtained within 3 years of hire)
National certification in care management or specialty area
At least 1 year of care management or utilization review experience OR completion of TIPS / Case Management Orientation Program
