Job Summary:

The RN performs under the direct supervision of the Patient Care Manager. The RN shall implement the nursing process in an organized, systematic manner to include assessment, planning, intervention, and evaluation in collaboration as evidenced by documentation in Patient Care Record, direct observation by Supervisor, and patient evaluations of care. The RN is responsible for working with all Forrest Health Facilities in transferring patients to and from the system, triaging and prioritizing patient transfers as appropriate based upon acuity and available resources at the facility, following transfer protocols and policies, teamwork within the department to optimize efficiency, be able to function as the Patient Placement Specialist and assign bed placement as appropriate, and consult and maintain the hospitalist as appropriate. Performs other job duties as assigned.

Performance Expectations:

Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed.

Demonstrates thorough, concise and timely documentation, shift reports and verbal communication. Communicates pertinent patient information to all health care team members.

Demonstrates critical thinking skills and uses professional judgement to respond promptly in assisting proper transfer of patients.

Communicates effectively with others in an appropriate and timely manner. Adheres to and promotes the established values of the organization, i.e. customer service, safety, infection control and compliance standards.

Consistently follows patient care and FGH policy, procedures, and standards of care in performing nursing duties.

Communicates with facilities to provide a smooth transfer process for patients being received to FGH or being sent to another facility. Collects the appropriate data for transfer; including demographics and report of patient's current physical condition, vital signs and accepting physician.

Must have a general knowledge of the EMTALA laws.

A general knowledge of the facilities within and surrounding our service area and the resources available to those facilities.

Teamwork within the department and the regional facilities is essential to the efficiency of this department.

Must be able to maintain a working and professional relationship with providers, other facilities, and departments.

Qualifications:

Education/Skills:

Graduate of a recognized non-online RN program required.

Work Experience:

One year medical/surgical nursing care preferred, but not required.

Certification/Licensure-DUE UPON HIRE

Licensed RN able to practice within the State of MS

Additional Certification/Licensure - Obtained based on required timeframe below

Basic Life Support

Within 30 Days of Employment

NIH Stroke Scale

Within 90 Days of Employment
