Ayden Healthcare of Wauseon is currently seeking a Social Service / Admissions Director

The Social Services Director is responsible for assisting in planning, organizing, implementing, evaluating and directing the Social Services Department in accordance with current existing federal, state & local standards, as well as established facility policies and procedures, to ensure that the medically-related emotional and social needs of the patient/resident are met/maintained on an individual basis .

*The Social Services Director will also meet with administration, medical and nursing staff and any other related departments in planning social services.

*Qualifying candidate will maintain a good rapport with intra-department personnel,other depts. in facility, & outside community health, welfare and social agencies, to ensure that social service programs are properly maintained to meet the needs of patients/residents.

*Interviews patients/residents/families as necessary in private setting

*Provides information to patients/resident/families regarding Medicare/Medicaid and other financial assistance programs available.

*Maintains confidentiality of all patient/resident health information to ensure patient/residents rights are protected

*Evaluates social and family information and assists in determining care plans for social treatment

The Admissions Coordinator will act as the initial contact as necessary with discharge planners, family or responsible parties to initiate tour of facility and follow up procedures.

The candidate will manage the admissions process to include:

* Effective Tours

* Timely completion of admission paperwork

* Coordinate clinical & financial approvals for admission

* Communicate with families and staff to ensure admission process is seamless
