RN Triage - Transitional Care Clinic

Hourly Pay Range:

$33.96 - $52.64 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights:

Position: RN Triage - Transitional Care Clinic

Location: 120 Spalding Drive, Naperville

Full Time 40 hours per week

Hours: M-F 8:00-4:30 - no evenings no weekends

Required Travel: No

What you will do:

Room patient, record vital signs and brief history from patient.

Completes patient admission assessment, including database and medication list

Provides SBAR handoff for APP and pharmacist

Ability to work as assigned direct care nurse.

Completes patient discharge process, including After Visit Summary, and ensures medication reconciliation completion.

Accepts other RN responsibilities as requested by the Manager of TCC.

Reports to Manager, Transitional Care Clinic for initial and subsequent assignments.

What you will need:

Bachelors Degree Science - Preferred

1 Year of telephone triage experience preferred, preferably in medical practice setting - Preferred

Registered Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR) Current State of Illinois Registered Nurse licensure - Required

American Red Cross CPR BLS or issued by American Heart Association - Required
