In order to submit a completed application please first connect with our Student Intern Coordinator ; Dulce Ruiz at regarding the request form that will to be uploaded to your application.

This intern substitute certificate will allow you to substitute for your mentor teacher ONLY, during the duration of your student teaching.

It cannot exceed a total of 12 days and is limited to a maximum of three consecutive occurrences.

Please also adhere to your university policies regarding when to apply for this certificate.
