RN Diabetes Care Manager, Program Coordinator No Weekends, No Holidays

Department: Diabetes Education

Hours: 32.00 per week

Shift: Shift 1

Responsible for the coordination of all aspects of the Diabetes Education Program with respect to patient care and maintaining ADA accreditation for the program.

The RN Diabetes Care Manager is competent with the knowledge and skills to create individualized self-management action plans, coordinate care, interpret clinical data, conduct focused and or complete educational assessments for patient population (diabetes).

Minimum Qualifications:

A graduate of an accredited Bachelor of Nursing program and registered nurse with an active license to practice nursing in CT.

Seven (7) years of clinical practice.

Preferred Qualifications:

Masters in Nursing, Certified Diabetes Care and Education Specialist (CDCES) or experience in education

or willingness to meet requirements to begin CDCES certification process within 6 months of hire is required and case/care coordination management experience is desired.
