What You'll Do

Round regularly and manage wound care for patients at assigned skilled nursing facilities (SNFs).

Collaborate with wound care nursing staff to assess and treat residents with wounds.

Deliver high-quality, patient-centered, comprehensive wound care services.

Complete timely and accurate documentation for all patient encounters.

Be available for phone and secure messaging consultations with facility nursing staff.

Document care in Theoria's ChartEasy™ EHR and interface with facility EHRs (PointClickCare, MatrixCare, American HealthTech).
