Position Overview: Join the dedicated Palliative Care team at The Hospital of Central Connecticut as an Advanced Practitioner! We are looking for a skilled Nurse Practitioner or Physician Assistant to provide compassionate and expert care for patients facing chronic, serious, or life-threatening illnesses. This role focuses on enhancing the quality of life for patients and their families, particularly those nearing the end of life, through skilled pain and symptom management.
Your Role: You will work collaboratively with the attending physician and an interdisciplinary care team to formulate comprehensive care plans that address not only medical needs but also emotional, economic, and cultural factors that can impact patient care.
Location: 435 Lewis Avenue, Meriden, CT 06451
Schedule: Three 8-hour weekday shifts, with specific days to be negotiated.

Role Scope: In this role, you will conduct physical examinations, develop medical and psychosocial treatment plans, and coordinate care with specialists as necessary. You will manage treatment within your scope of practice and identify when referrals to a physician are appropriate.
Qualifications:
Licensure: Unrestricted State of Connecticut Medical License (or eligibility).
Training: Completion of a Nurse Practitioner or Physician Assistant program.
Experience: 1+ years of experience preferred; National Board Certification in Palliative Care is a plus.
