We are looking for a Social Service Coordinator who can help support the physical, emotional, and social needs of our residents. This role is a key part of our care team-helping create a welcoming, person-centered environment, completing psychosocial assessments, coordinating needed services, and supporting residents in reaching their highest level of well-being. The director also guides the day-to-day operations of the social services department and its programs.

Qualifications:

current social work certification in the state of Wisconsin

excellent communication and listening skills

ability to organize and prioritize effectively

bachelor's degree in social work or related health care field is preferred
