Your Role as a Physical Therapist:

Evaluate, plan, and implement personalized therapy for residents with temporary or long-term physical conditions.

Provide treatments in line with the plan of care and modify as needed based on resident progress.

Educate and support patients, families, and caregivers throughout the rehab journey.

Monitor and document patient responses, progress, and outcomes using the Net Health electronic documentation system (preferred).

Collaborate with our multidisciplinary team to promote residents’ healing, independence, and overall quality of life.

What We’re Looking For:

Current Physical Therapist license in the State of Pennsylvania (in good standing).

SNF/Long-Term Care experience preferred.

Strong communication and interpersonal skills.

Familiarity with modern PT practices and documentation systems.

Compassionate, patient-centered mindset.
