IV Pharmacist Job Description We are seeking a dedicated and detail-oriented IV Pharmacist to join our team in a dynamic long-term care environment.
This role involves ensuring the accuracy and safety of medication dispensing, maintaining comprehensive patient records, and collaborating with healthcare professionals to optimize patient care.
Responsibilities * Review medication orders for accuracy, appropriate dosing, potential interactions, and proper administration.
* Ensure medications are dispensed correctly and labeled accurately.
* Communicate with nursing staff, physicians, and other healthcare professionals regarding medication-related inquiries.
* Oversee medication preparation processes.
* Recommend therapy adjustments when necessary.
* Investigate and resolve medication-related issues promptly.
* Maintain patient records, including medical history, clinical data, and lab results.
Essential Skills * Active Ohio Pharmacist license required.
* Strong analytical and problem-solving skills.
* Exceptional attention to detail and accuracy.
* Ability to work independently and manage multiple tasks efficiently.
Additional Skills & Qualifications * Previous experience in long-term care pharmacy preferred.
* Proficiency with pharmacy software systems (knowledge of Frameworks LTC is a plus). Work Environment Work in a long-term care facility with shifts every other week, Monday through Sunday, from 8:30 PM to 7:00 AM.
