Job Responsibilities

Provide skilled physical therapy assessments and treatments to home health patients

Develop personalized treatment plans and adjust based on patient progress

Manage 5-6 patient visits per day within a 30-mile radius of Jackson, CA

Educate patients and families on mobility, strength, and pain management

Document patient progress accurately in the EMR system (Matrix)

Collaborate with healthcare teams to coordinate care

Ensure compliance with all state and federal healthcare regulations

Maintain professionalism and compassionate patient interactions
