Since 1960, D’Youville Life & Wellness Community has provided Greater Lowell and surrounding communities with Faith-Based, Nonprofit, Short-Term Rehabilitation, Skilled Geriatric Care and Assisted Living Facility earning the respect of its peers and the trust of the community at large.

We are looking for a Wellness Nurse - Assisted Living Community - RN - Registered Nurse or LPN - Licensed Practical Nurse to join our dynamic team.

Per Diem Shifts - MUST BE ABLE TO WORK: Every Other Weekend.

Pay Range: $33 to $39.60 base pay based on license and experience. Shift Differential added to evening shift and weekend shift.

Wellness Nurse - Assisted Living Community - RN or LPN Qualifications:

MA RN - Registered Nurse or LPN - Licensed Practical Nurse License.

1 year of Assisted Living Facility experience preferred.

Wellness Nurse - Assisted Living Community - RN or LPN Job Summary:

Implement and coordinate resident care in collaboration with physician and nursing staff.

Document all resident care information.

Provide functional direction and assistance to nursing support staff.

Complete new staff orientation as assigned.

Direct resident care to include making round, answering call lights, assist with medications.

Complete monthly/quarterly audits, assessments and documentation.

Communicate effectively with the residents, families, visitors, members of the health care team, and other disciplines.

Participate in screening potential admissions, as needed.
