About Total Care Connect

Total Care Connect (TCC) is a mobile integrated health organization delivering in-home clinical and preventive care to members across Ohio and surrounding regions. We support health plans, health systems, and value-based organizations by reaching members where they are — in their homes and communities — to improve access, close care gaps, and reduce avoidable utilization.

As a tech-enabled, field-based care delivery organization, our teams provide a range of services including preventive care, chronic condition support, transition-of-care visits, member engagement, and navigation. We operate with a focus on high-quality member experience, operational excellence, and coordinated care across clinical, administrative, and remote teams.

About the Role

We are seeking a Head of People & Culture to build and lead TCC’s People function as we continue to grow. This is a hands-on leadership role responsible for HR operations, recruiting, employee experience, and culture across our clinical teams, administrative staff, and remote workforce.

The ideal candidate brings strong HR fundamentals, healthcare experience, and the ability to build scalable people systems in a fast-paced, high-growth organization.

Compensation & Benefits

Salary: $90,000–$105,000

Bonus: Annual performance-based

Key Responsibilities

People & Culture Leadership

Develop and lead TCC’s People & Culture strategy across clinical, non-clinical, and remote teams

Promote a cohesive, mission-driven culture aligned with field-based care delivery

Support communication, engagement, leadership development, and internal alignment

HR Operations & Compliance

Oversee HR operations including onboarding, offboarding, employee files, benefit administration, ADP Workforce Now, and policy development

Maintain compliance with state and federal employment regulations as we expand into new markets

Update employee handbook, HR SOPs, and documentation practices

Talent Acquisition & Onboarding

Lead and support recruiting efforts for clinical, operational, and administrative roles

Implement structured interview processes, job scorecards, and consistent hiring workflows

Ensure high-quality, consistent onboarding experiences across all departments

Employee Relations & Manager Support

Serve as a trusted advisor to leadership, managers, and team members

Support employee relations, performance discussions, and conflict resolution

Provide coaching and training to strengthen management capabilities

Organizational Planning

Partner with leadership on future org structure, role clarity, and workforce planning

Support performance management systems and employee development initiatives

Requirements

6–10+ years of HR, People Operations, or related experience

Healthcare experience required (value-based care, care delivery, managed care, home-based care, clinical operations, health system, or similar)

Bachelor’s degree in human resources, healthcare administration, or related field.

SHRM certification preferred

Experience supporting distributed or multi-site teams (clinical + administrative + remote)

Strong foundation in HR operations, talent acquisition, and employee relations

Hands-on builder with ability to create scalable systems in a growing organization

Excellent communication, interpersonal, and leadership skills
