Job Title: RN - Case Manager

Location: Columbus, OH 43215

Duration: 3+ Months (Possibility of Extension)

Shift: Monday - Friday, 8:00 AM - 5:00 PM EST

Pay Rate: $34.12/hr. on W2

Position Summary: The RN Case Manager employs a collaborative approach involving assessment, planning, facilitation, and advocacy to coordinate services that align with an individuals benefit plan and healthcare needs. Utilizing effective communication and resource management, the Case Manager supports optimal, cost-effective health outcomes for patients.

Required Qualifications:

Active, unrestricted Registered Nurse (RN) license.

Clinical experience in hospital, home health, ambulatory care, or other alternative care settings.

Preferred Qualifications:

Prior case management experience.

Experience within healthcare or managed care environments.

Case Management Certification (CCM) preferred.
