As a Nurse Practitioner, you will perform annual risk assessments in patients' residences or via telehealth, aligning with the scope of practice delegated by the Collaborative Physician.
Essential Duties and Responsibilities
Conduct thorough in-home health risk assessments to identify active and chronic disease conditions, and evaluate physical, mental, and social needs during each visit.
Perform patient histories, examinations, and determine diagnoses.
Document patient visits clearly and accurately, in accordance with NCQA standards.
Measure patient vital signs as needed.
Initiate case management referrals and communicate with Primary Care Physicians (PCP) when necessary.
Engage with patients, caregivers, agency nurses, other providers, and vendors to ensure proper diagnoses and care coordination.
Observe OSHA Universal Precautions while performing clinical duties.
Maintain confidentiality of patient information.
Attend required meetings, in-services, and participate in committees as requested.
Engage in professional development activities to maintain licenses and affiliations.

Qualifications
Required Knowledge, Skills, and Experience
Active/unrestricted nurse practitioner license for states of practice.
Board certified by ANCC, AANP, or NCCPA.
Current CPR Certification.
Enrollment in Medicare/Medicaid programs.
Valid driver's license with a clean driving record.
Excellent electronic health record (EHR) skills.
Preferred Knowledge, Skills, and Experience
Experience or training in geriatrics.
Ability to work effectively as part of a team with patients, medical staff, and the public.

Ability to adapt to various weather conditions and diverse patient residential environments.
Full range of motion including the capability to handle/lift patients, manual dexterity, normal visual and auditory acuity, and physical endurance such as standing and climbing stairs.
Regularly lift/carry items weighing up to 50 pounds.
Willingness to travel up to 150 miles daily in both urban and rural settings; must be able to drive if necessary.
