This role offers a unique opportunity to work in our Meng Residence, a home-like hospice setting where you can slow down, build meaningful connections, and provide truly patient-centered care. This setting allows you to focus on quality, presence, and comfort for both patients and their families.

Schedule: Primarily based at the Meng Residence in De Pere, working 32–36 hours per week, 9:00 am to 9:00 pm shifts. There will be occasional 3:00 pm to 11:00 pm shifts in the community to support patient care needs, along with an every third weekend rotation and 2-3 holidays per year.

What You’ll Do:

Provide hands-on, compassionate care to patients in a home-like hospice setting

Support and follow individualized care plans in collaboration with the RN and care team

Monitor patient conditions and communicate any changes promptly

Provide education and support to patients and families

Assist with clinical tasks, including IV monitoring under RN direction

Document care accurately and maintain compliance with regulatory standards

Help create a calm, respectful, and supportive environment

We are looking for someone who brings both clinical skill and a strong sense of compassion. Experience in long-term care, hospice, or a similar setting is helpful, along with the ability to adapt between a residential setting and occasional community support.
