Title IV-E Program Coordinator (Human Services Program Consultant)

DYS is seeking a Title IV-E Program Coordinator [Human Services Program Consultant] in our Office of Community Based Innovations & Solutions.

Duties include, but are not limited to: * Acts as program consultant and liaison for youth assigned to Title IV-E facilities.

* Maintains accurate and up-to-date case records in SACWIS and electronic case files for assigned youth.

* Coordinate case transfers and eligibility cases between county and state agencies and IV-E facilities.

* Conducts family assessments, case plans & case reviews for assigned youth.

* Follows state and federal guidelines to establish youth IV-E eligibility and completes determination.

* Participates in CANS and QRTP hearings when required.

* Coordinate with Ohio Department of Medicaid to ensure youth in placement have medicaid coverage and update SACWIS.

* Enter and update indipendent living plans for all IV-E youth in SACWIS * Coordinates graduation allocation eligibility and fund distribution with IV-E placements, Department of Children and Youth, and DYS fiscal staff.

Completion of undergraduate core program in social or behavioral science, health or pre-medicine or comparable field; 18 mos.

in delivery of human services, health care services or medical assistance in governmental, community or private human support services agency or medical provider.

* Or completion of graduate core program in social or behavioral science or medicine-related or comparable field; 6 mos.

* Or equivalent of Minimum Class Qualifications for Employment noted above.
