Salary: $45,000.00 To $60,000.00 Annually

We are currently seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team.

Position: Licensed Practical Nurse (LPN) / PDN Nurse

Qualifications

Graduate of an accredited Practical/Vocational Nursing program

Minimum of two (2) years of experience in a home health care setting

Knowledge of state and federal regulations

Strong oral and written communication skills

Solid understanding of patient care and practical nursing procedures

Ability to assess, prioritize, and follow up on patient care needs

Ability to work independently and as part of a multidisciplinary team

Ability to perform physical tasks, including standing, bending, lifting, and moving patients/equipment (50+ lbs)

Duties & Responsibilities

Practice within the LPN scope as defined by the State Board of Nursing

Maintain patient dignity, privacy, and confidentiality

Perform treatments as ordered by a physician under RN supervision

Administer medications within the scope of practice

Collaborate with healthcare team members and educate patients/families

Accurately document and report patient care, observations, and changes to the RN

Perform additional duties as assigned
