XstremeMD is seeking a bilingual (English/Spanish) Licensed Practical Nurse (LPN) Case Manager to coordinate and monitor medical care for injured or ill individuals. This role involves working closely with employers, healthcare providers, insurance carriers, and employees to ensure appropriate treatment, effective communication, and a safe return to work. Candidates must have a current LPN license, strong organizational and communication skills, and the ability to manage multiple cases in a fast-paced environment. Fluency in both Spanish and English is required to support our diverse workforce. Requirements:

LPN with current, valid state licensure required

Must be fluent in Spanish and English

Minimum of 2+ years clinical nursing experience required

Minimum of 2+ years Emergency Room/ICU experience preferred

Case Management and Workers compensation-related experience preferred

Excellent interpersonal skills, phone etiquette, time management and organizational skills are essential

Must be proficient in Microsoft Office with programs such as Word and Excel, as well as have experience with Internet searches and a comfort level with other software programs

Must be able to type 50 wpm minimum

Ability to set priorities and work independently is essential

Must be able to pass a background check and drug test, as applicable for the role
