Position Highlights

Deliver comprehensive, patient-centered care for a senior-only panel

Monday to Friday schedule, 8:00 AM to 5:00 PM with a dedicated lunch hour

Patient volume: 10-15 per day, allowing for quality, unrushed visits

Focus on preventive care, chronic disease management, and patient education

Close collaboration with supervising physicians and a multidisciplinary care team

Practice medicine in a model designed around outcomes, not volume

Candidate Requirements

Graduate of an accredited Nurse Practitioner program with active licensure

Bilingual (Spanish) required to serve the local community, encouraged to still apply for other nearby positions if interested

Confident managing chronic conditions such as diabetes, COPD, CHF, and hypertension

Skilled in EMR documentation and committed to accuracy

Compassionate, proactive, and patient-centered in approach

Strong clinical judgment with the ability to think on your feet

Excellent communication skills both with patients and team members
