Position: Licensed Practical Nurse ( LPN )

Shift: Day Shift 7AM3PM Full-Time

Hourly Pay Range: $24.00$31.45/hour (based on experience)

Sign-On Bonus: $10,000

Pos ition Summary: Licensed Practical Nurse ( LPN )- Day Shift

We areseekinga skilled and compassionate Licensed Practical Nurse (LPN)to join our team. The LPN will provide direct nursing care to residents,assistwith rehabilitation plans, and collaborate with RNs, physicians, and other healthcare professionals to ensure the highest standards of care in our long-term care and rehabilitation facility.

Qualifications:

Current and valid LPN license

Strong clinical skills, including medication administration and wound care.

Excellent communication and interpersonal skills.

Ability to work effectively in a team-oriented environment.

Compassionate, patient-focused approach to care.

BLS/CPR certificationrequired.

Responsibilities:

Administer medications and treatmentsas prescribed byphysicians.

Monitor and document residents health status, including vital signs, and report changes to the supervising RN or physician.

Assistwith activities of daily living (ADLs), including bathing, dressing, and feeding, as needed.

Collaborate with the rehabilitation team to support residents recovery goals, including physical, occupational, and speech therapy plans.

Maintainaccurateandtimelydocumentationin accordance withfacility policies andstate/federal regulations.

Provide emotional support and education to residents and their families.

Ensure a safe and clean environment, adhering to infection control protocols.
