Contract Type: Travel Contract
Shift Type: Full-time PM shift, 2:00 PM - 10:00 PM, includes every other weekend (evening/PM)
Hours per Week: 36
Candidate Pay Rate: $45 per hour

Roles & Responsibilities
Administer medications safely and accurately following the 5 Rights to support resident well-being
Assess residents and report changes in condition to the charge nurse and physicians promptly
Document clinical observations and communicate updates to families and appropriate staff
Assist CNAs with resident care and respond to call lights as part of a collaborative team
Report any allegations of abuse, neglect, or misappropriation according to facility protocol
Support the nursing team by performing other duties as requested to ensure quality resident care
Qualifications
Current Wisconsin Licensed Practical Nurse (LPN) license required
Current CPR/BLS certification required
Minimum 1+ year of related LPN experience preferred
Strong accuracy and attention to detail in medication administration and documentation
Comfortable communicating with physicians, families, and interdisciplinary care teams
Ability to follow long term care policies and report changes or concerns promptly
Education
LPN diploma, certificate, or equivalent vocational nursing education required
Knowledge & Skills
Medication administration and resident assessment skills
Knowledge of long term care (LTC) practices and regulatory reporting
Skilled at clear, compassionate communication with residents and families
Attention to detail for documentation and reporting changes in condition
Teamwork mindset and ability to assist CNAs and nursing staff as needed
