Job Overview

We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. The ideal candidate will have a strong background in primary care and hospital rounding, as well as a passion for providing high-quality patient care. As an FNP, you will be responsible for assessing, diagnosing, and treating patients of all ages, while also collaborating with other healthcare professionals to ensure comprehensive care.

Responsibilities

Conduct thorough physical examinations and health assessments of patients.

Develop and implement individualized care plans based on patient needs.

Perform diagnostic evaluations and interpret lab results.

Provide education and counseling to patients regarding health management and disease prevention.

Manage acute and chronic conditions in diverse populations, including children and seniors.

Collaborate with interdisciplinary teams to coordinate patient care in various settings such as outpatient clinics, hospitals, and assisted living facilities.

Maintain accurate medical records and documentation in compliance with HIPAA regulations.

Experience

Candidates should possess the following skills and experiences:

Familiarity in EMR/EHR systems such as Kareo, Tebra, Wellsky, Epic, Athenahealth or Cerner.

Experience in primary care or urgent care settings.

Familiarity with CPT coding and ICD-10 coding.

Familiarity with geriatric population and special needs population is desirable.

Familiarity with telehealth practices and home care services is a plus.
